Terms & Conditions

These Terms & Conditions ("Terms") govern all purchases, memberships, bookings, services, and participation with Forma Haus LLC ("Forma Haus," "we," "us," or "our"), which operates under the Forma Haus Lagree brand. By purchasing, booking, enrolling in, or participating in any class, membership, package, workshop, event, retail purchase, gift card, or other product or service offered by Forma Haus Lagree, you acknowledge that you have read, understood, and agree to be bound by these Terms.
1.Purchases & Payment
All prices are listed in U.S. Dollars (USD) and are subject to change at any time without notice.
Full payment is required at the time of purchase unless otherwise stated in writing by Forma Haus Lagree.
We accept approved payment methods made available at checkout.
You authorize Forma Haus Lagree to charge your selected payment method for all purchases, recurring membership charges, applicable taxes, fees, penalties, and other authorized charges.
You are responsible for maintaining current and accurate payment information at all times.
2.Classes, Packages & Memberships
Class packs, memberships, promotional offers, and account credits are non-transferable and may only be used by the purchasing client unless expressly stated otherwise.
Expiration dates apply to all class packs, promotions, introductory offers, and account credits and will not be extended except where required by law.
Membership benefits may only be used by the registered member.
Unused classes, credits, or membership benefits do not carry cash value and may not be redeemed for cash.
Memberships automatically renew according to their applicable billing cycle unless canceled in accordance with these Terms.
3.Class Reservations, Cancellations, No-Shows & Late Arrivals
Clients are responsible for managing their reservations through the designated booking platform.
Class cancellations must be made at least eight (8) hours prior to the scheduled class start time.
Late cancellations will incur a $10 fee and result in the loss of the class credit.
No-shows will incur a $20 fee and result in the loss of the class credit.
Late arrivals may be denied entry at the discretion of studio staff or instructors for safety, operational, or class experience reasons and may be treated as a no-show.
Forma Haus Lagree reserves the right to modify cancellation fees and policies upon notice.
4.Membership Cancellation, Auto-Renewal & Commitments
Memberships may be canceled with at least fourteen (14) days' written notice prior to the next billing date.
By purchasing a membership, you acknowledge and agree that your membership will automatically renew at the end of each billing period unless canceled in accordance with these Terms. Recurring membership charges will automatically be billed to the payment method on file at the then-current membership rate.
By enrolling in a membership, you authorize recurring charges to your payment method until your membership is properly canceled in accordance with these Terms.
Memberships must complete their original commitment term.
If proper cancellation notice is not provided, the membership will automatically renew according to its billing cycle and applicable commitment structure.
Early cancellation prior to completion of the original commitment term will incur a $200 early cancellation fee.
Membership freezes are permitted only in cases of verified medical necessity and require submission of a physician's note or other documentation acceptable to Forma Haus Lagree.
Members are eligible for a maximum of one (1) approved membership freeze per membership commitment term.
Approved membership freezes will be subject to a $50 monthly freeze fee and may not exceed three (3) consecutive months.
During an approved freeze period, membership benefits, class access, and booking privileges will be suspended. Membership billing will be adjusted to the applicable freeze rate.
At the conclusion of the approved freeze period, the membership will automatically reactivate and regular membership billing will resume using the payment method on file.
The duration of any approved freeze will extend the member's commitment term by the same amount of time as the freeze period.
Freeze requests must be submitted in writing and approved by Forma Haus Lagree prior to the requested freeze start date. Retroactive freezes will not be granted.
No other pauses, freezes, suspensions, transfers, or holds will be permitted except at the sole discretion of Forma Haus Lagree.
Cancellation requests must be submitted in writing to hello@formahauslagree.com and are not considered effective until confirmed in writing by Forma Haus Lagree.
Declined, expired, or failed payment methods do not constitute cancellation of a membership.
Forma Haus Lagree reserves the right to suspend booking privileges, restrict account access, pursue collection efforts, or assess applicable fees for unpaid balances.
All membership payments are non-refundable except as required by law.
5.Refunds, Credits & Exchanges
All purchases are final sale.
No refunds, credits, exchanges, or transfers will be provided for classes, memberships, packages, workshops, events, or services except as required by applicable law.
All retail merchandise is final sale and is non-refundable and non-exchangeable.
Gift cards are non-refundable, non-transferable, have no cash value, and may not be redeemed for cash except where required by law.
In the event Forma Haus Lagree is unable to provide services due to circumstances beyond its reasonable control, Forma Haus Lagree may, at its sole discretion, provide credits, extensions, prorated adjustments, alternative services, or other remedies.
6.Gift Cards & Promotional Offers
Gift cards may only be redeemed toward eligible Forma Haus Lagree products and services.
Promotional offers, discounts, introductory packages, and incentives may not be combined unless expressly stated otherwise.
Promotions may be modified, suspended, or discontinued at any time.
Promotions and discounts have no cash value and may expire.
7.Health & Safety Acknowledgment
You acknowledge that participation in fitness activities, exercise classes, and use of fitness equipment involves inherent risks, including but not limited to injury, illness, disability, property damage, or death.
You represent that you are physically capable of participating in classes and activities offered by Forma Haus Lagree.
You agree to consult a physician before participating if you have any medical concerns, conditions, injuries, or limitations.
You agree to immediately discontinue participation and notify staff if you experience pain, dizziness, discomfort, or any medical issue during participation.
8.Assumption of Risk & Liability Waiver
You voluntarily assume all risks associated with participation in classes, events, workshops, use of facilities, and use of equipment.
To the fullest extent permitted by law, you release, waive, discharge, and hold harmless Forma Haus LLC, its owners, officers, directors, employees, instructors, contractors, agents, successors, affiliates, and representatives from any claims, liabilities, damages, losses, costs, expenses, or causes of action arising out of or relating to participation in any activity or use of any services, facilities, or equipment.
Participation may require execution of a separate Liability Waiver and Release Agreement, which is incorporated into these Terms by reference.
9.Studio Policies & Client Conduct
Clients are expected to follow all studio policies, posted rules, instructor guidance, and safety procedures.
Disruptive, abusive, threatening, inappropriate, unsafe, or unlawful conduct may result in removal from class, suspension of privileges, termination of membership, or refusal of future service without refund.
Forma Haus Lagree reserves the right to refuse or terminate service for violations of studio policies, safety concerns, operational reasons, or other lawful grounds.
10.Changes to Schedule, Instructors & Services
Class schedules, instructors, formats, amenities, services, operating hours, and offerings are subject to change at any time.
Forma Haus Lagree does not guarantee the availability of any specific instructor, class format, service, or schedule.
Forma Haus Lagree shall not be liable for cancellations, substitutions, delays, interruptions, or schedule changes.
11.Force Majeure
Forma Haus Lagree shall not be liable for any delay, interruption, modification, reduction, suspension, or failure to perform due to causes beyond its reasonable control, including but not limited to natural disasters, severe weather, fire, flood, earthquake, pandemic, epidemic, public health emergency, government action, labor disputes, utility outages, equipment failures, civil unrest, acts of terrorism, supply chain disruptions, or other force majeure events.
12.Intellectual Property
All trademarks, logos, branding, class concepts, videos, photographs, graphics, website content, materials, and intellectual property associated with Forma Haus Lagree are owned by or licensed to Forma Haus Lagree.
No content may be copied, reproduced, distributed, recorded, published, or used without prior written permission.
13.Privacy
Personal information collected through purchases, bookings, memberships, communications, or participation is handled in accordance with our Privacy Policy.
We do not sell personal information except as permitted by applicable law or as necessary to process transactions, provide services, comply with legal obligations, or protect our rights.
13A. SMS Messaging Terms
Program DescriptionBy providing your mobile phone number and opting in to receive text messages from Forma Haus Lagree, you agree to receive recurring automated text messages from Forma Haus LLC ("Forma Haus Lagree," "we," "us," or "our") at the mobile number you provide. Messages may include appointment and class reminders, booking confirmations, waitlist notifications, membership and account updates, billing notifications, promotional offers, marketing communications, customer support follow-ups, and other information related to our services. Consent to receive text messages is not a condition of any purchase.
Message FrequencyMessage frequency may vary depending on your account activity, bookings, memberships, preferences, and interactions with Forma Haus Lagree. You may receive up to ten (10) messages per month, although actual message volume may vary.
Message and Data RatesMessage and data rates may apply. Charges are billed by and payable to your mobile service provider. Please contact your wireless carrier for information regarding your messaging and data plans.
How to Opt Out and Get HelpYou may opt out of receiving SMS messages at any time by replying STOP to any text message we send. After you send STOP, you will receive a one-time confirmation message and will no longer receive SMS messages from us unless you subsequently opt in again. To rejoin, you may sign up again through the original opt-in method or reply START if available.

Carrier Liability DisclaimerWireless carriers are not liable for delayed, undelivered, misdirected, or failed messages.
PrivacyInformation collected through our SMS program is subject to our Privacy Policy. We may use information provided through the SMS program to deliver messages, administer the program, provide customer support, and otherwise operate our business in accordance with applicable law.
14.Photography & Media Release
Forma Haus Lagree may photograph, record, livestream, or otherwise capture images, video, or audio within its facilities and during classes, events, and activities.
By entering the studio or participating in services, you grant Forma Haus Lagree permission to use your likeness, image, voice, name, and appearance in photographs, videos, recordings, and promotional materials without compensation.
Clients who do not wish to appear in promotional materials must provide written notice prior to participation.
15.Minor Participants
Participants under eighteen (18) years of age may only participate with the written consent of a parent or legal guardian and must have all required agreements, waivers, and releases executed by a parent or legal guardian.
16.Electronic Consent & Agreement
By creating an account, making a purchase, booking a class, enrolling in a membership, checking a box indicating acceptance, signing electronically, or otherwise using Forma Haus Lagree services, you acknowledge and agree that:
(a) You have read, understood, and agree to these Terms & Conditions, the Liability Waiver and Release Agreement, Privacy Policy, and all studio policies.
(b) Your electronic acceptance constitutes your legal signature and has the same force and effect as a handwritten signature.
(c) You consent to receive communications electronically, including billing notices, membership notices, account updates, and policy changes.
(d) Electronic records maintained by Forma Haus Lagree shall be deemed valid business records to the fullest extent permitted by law.
17.Governing Law
These Terms shall be governed by and construed in accordance with the laws of the State of California without regard to conflict of law principles.
Any dispute arising out of or relating to these Terms, services, or participation shall be resolved in the courts located in Ventura County, California.
18.Amendments
Forma Haus Lagree reserves the right to update or modify these Terms at any time.
Material changes will be posted on our website and will become effective upon posting unless otherwise stated.
Continued purchases, bookings, memberships, or participation following any changes constitutes acceptance of the revised Terms.
